Acne scars are the leftover signs of your immune system’s attempt to heal lesions caused by shear pressure on the cells lining the sebum canals, inside the hair follicles where the sebaseous glands reside or by attempting to heal lesions caused by chemicals produced by uncontrolled bacteria that damage the cells and initiate an inflammatory reaction.

This happens whenever the flow of sebum is suddenly abundant and/or it is impeded to outlflow swiftly to the surface of the skin. Specifically because of one of three possible reasons:

1.- Plugs that block pores, the visible part of the ducts on the surface of the skin. 2.- Damage to the cells that form the walls of the sebum channels inflicted by sudden outlflow of large quantities of sebum that exerts pressure on the follicular lining in the pilosebaceous unit that those cells can not withstand, or

3.- Lesions to the follicular cells by chemicals secreted by Propionibacteria acnes bacteria, that are normally resident and live in symbiosis on the keratin lined follicular canal, helping the sebaseous glands transform dead cells into sebum, when those bacteria proliferate uncontrolled due to an excessively rich feeding environment composed of the same sebum that is trapped and can not freely outflow to the skin surface.

If the sebum outflows to the surface of the skin swiftly, where it plays a skin protective role against other types of bacteria living on the surface of the skin, no lesions are inflicted to the delicate cells lining the sebum canals or ducts and no acne bacteria proliferates inside the sebum canals beyond the point where the chemicals they secrete causes damage to those walls/cells. Then, there is no lesions to which your immune sytem needs to react as if it where a threat where life and death was at stake.

• Propionibacteria acnes are anaerobic bacteria that find oxygen toxic and inhabit the hair follicles. Its growth is supported by sebum. Produces propionic acid which maintains a low pH of skin and has bacteriostatic effect (limits reproduction) on many potentially harmful microbes.

• Propionibacteria acnes lives in symbiosis on the keratin lined follicular canal. Proprionibacterium acnes ingests the sebum produced from the sebocytes of the sebaceous glands. This nascent sebum is largely lipid in composition and also contains DNA, RNA, proteins and other cellular components that result from the breakdown of sebocytes themselves. The Proprionibacterium acnes which are highly lipophilic, feed on the nascent sebum. Proprionibacterium acnes are found only in sebaceous rich areas. If the nutrients increase due to an active and large sebaceous system, then colonization and high growth rates of Proprionibacterium acnes will form. The resident bacterial flora produces biologically active molecules such as histamine, extracellular enzymes and peptides which may be responsible for the chemical attraction (chemotaxis) of the inflammatory infiltrate in problematic acne vulgaris. If colonization of Proprionibacterium acnes occurs in sufficient numbers, they can produce initiating antigenic molecules that promote the initiation of inflammation. Proprionibacterium acnes can produce proteinases, lipase and hyaluronate lyase all of which may serve as the catalysts or initiators of the inflammatory infiltrate composed of neutrophils and lymphocytes. In technical words: Proprionibacterium acnes when gone uncontrolled may stimulate keratinocytes to produce proinflammatory cytokines.

Surgical methods of scar removal may be effective for some people who are willing to run the high risks of side effects. Infections and color changes in the skin are some of the most frequent outcomes. Chemical peels and dermabrasion procedures are yet another option. They are performed by trained professionals, which makes them an expensive treatment for acne scarring.

Since the procedure causes a new wound, there is always a chance of infection. 6 to 8 weeks is needed for complete healing so this can sway some people away from the surgery. Some deeper scarring requires different techniques so this is not for everyone.

Microdermabrasion is another skin resurfacing protocol that mends facial skin. A plastic surgeon uses a tool that looks like a sandblaster to spray tiny crystals across the face, mixing gentle abrading with suction to remove the pimple scarring. Like other scar removal techniques, it is possible that one session will not give you the desired results.

Microdermabrasion for acne scar removal can be used when the scar is raised above the surrounding skin, but is less effective with indented scars. People prone to keloid scars need to be very careful when using scrubbing methods for scar removal. Any cutting of keloids can cause further keloid growth.

Cosmetic laser resurfacing is used to refine acne scars and decrease skin redness around acne lesions. It is a controlled burning method, where a laser removes layers of skin, removing acne scarring and other impurities. This procedure creates a fresh skin surface by flattening out scars.
